Asset accountability forms: issuing company property to employees

What an asset accountability form should contain, how to issue and return company property, handling loss or damage fairly, and moving accountability forms online.

In Philippine companies, an accountability form (sometimes called a property acknowledgment or equipment issuance form) records that an employee received company property such as a laptop, phone, tools, uniform or vehicle, and accepts responsibility for it under company policy. It protects both sides: the company knows who has what, and the employee has proof of what was issued and in what condition.

Issuing property

  1. Prepare the item and record it in the asset system against the employee (asset check-in and check-out).
  2. Check and document its condition with the employee present.
  3. Explain the care and use policy briefly.
  4. Have the employee sign or confirm digitally.
  5. Give the employee a copy.

Returning property

When an employee returns property (at separation, transfer or end of use), check it against the original form: item, accessories and condition. Record the return and any issues the same day. At separation, the list of open accountabilities is a key part of clearance.

Loss and damage: handle it fairly

Policies should explain what happens when property is lost or damaged. Philippine labor rules allow wage deductions for loss or damage only under specific conditions: generally, the employee must be clearly shown to be responsible, be given a reasonable opportunity to explain, the amount must be fair and not exceed the actual loss, and deductions are limited in amount per pay period. Apply these consistently, document the process, and consult HR or legal counsel before deducting. Normal wear and tear is not the employee's responsibility.

Common problems with paper forms

  • Forms filed in branch folders, so head office cannot see who holds what
  • Serial numbers left blank, so items cannot be verified
  • Returns not recorded, so departed employees still appear accountable
  • No link between forms and the asset register

Moving accountability online

Recording issuance and returns in the asset system replaces stacks of paper forms:

  • Each assignment is linked to the employee and the asset record
  • Employees can see what is assigned to them
  • Clearance lists open accountabilities automatically
  • History of every handover is kept

Digital acknowledgment can replace signatures where company policy allows; keep the policy and acknowledgment records consistent.

Accountability for shared and pooled items

Not every item belongs to one person. For pooled tools, vehicles and equipment, use short-term check-out with due dates or asset booking so accountability is clear for each use.

Where Cirquolus fits

The Cirquolus asset management system records fixed assets, assignments, check-in and check-out and consumables per employee, and employees see their assigned assets and consumables in the employee portal. HRIS manages employee transitions so clearance includes open accountabilities.
